Transitional Care Program Nurse: From our office in Brecksville, Ohio, the Transitional Care Program Nurse will assess, plan, coordinate, monitor and evaluate patients requiring case management services within our transitional care program. This individual will work closely with the patient, the patient's caregivers and the payer to ensure the highest quality care for patients. Qualifications include current licensure as a Registered Nurse in Ohio, at least two years clinical experience in an acute care setting, and previous insurance or managed care experience. Qualified candidates must present outstanding communication skills, confidence in decision making, and superior organization skills. Preferred experience includes home health care and transition programs. Candidates must enjoy working collaboratively and must work effectively in a fast-paced environment. The Transitional Care Program Nurse will work Monday-Friday from 8:30am-5:00pm, but will be flexible to work a later shift two to three times per week, if our needs change in the future. Utilization Management Nurse: The UM Nurse provides service coordination for patients receiving home health and infusion services. UM Nurses work closely with the patient's caregivers and the payer to ensure the highest quality of care. Responsibilities: monitor case needs on an ongoing basis, obtaining clinical updates and insurance authorizations in a timely manner; clinically evaluate the plan of care as the case evolves and work collaboratively with the agency and payer to modify care approach as needed; document clinical information accurately and concisely. Qualifications: RN or LPN with current OH license; 2 years clinical experience in an acute care setting; Previous home health care, infusion therapy or insurance/managed care experience preferred; CCM certification a plus; outstanding communication skills; excellent organization reflected in work; intermediate computer skills, experience with windows, Microsoft office, and the internet. Admissions Coordinator: We are seeking a full-time Admissions Coordinator who will work to achieve timely, accurate completion of patient referrals. S/he will calculate patients' medication requirements in order to pre-certify patients for care and verify benefits. The Admissions Coordinator will interact regularly with payers, so candidates must be adept at developing and maintaining quality working relationships. An outstanding customer service presentation and strong communication skills are essential. The ability to multitask while completing work accurately is imperative, as this position works under pressure in a time-constrained environment. Candidates must excel working on a team, yet function independently at the highest level. Specific requirements include a high school diploma at minimum, mathematical aptitude - including the ability to calculate percentages and solve word problems, previous experience in healthcare, and intermediate computer experience. Collections Specialist: We are seeking a full-time Collections Specialist to join our reimbursement team! The Collections Specialist communicates with CSI staff and payors to obtain reimbursement for outstanding claims. Responsibilities include reviewing A/R reports to identify delinquent accounts, investigating delinquent accounts to determine a collectible balance, and working with payors to resolve outstanding receivables in a timely manner. This position also contacts our patients from time to time to verify current insurance coverage. Qualifications include a high school diploma or GED at minimum and excellent communication and time management skills. Candidates must be able to demonstrate past success paying careful attention to detail and completing work accurately. Previous healthcare reimbursement experience is preferred as is basic knowledge of the home health care industry. Candidates who enjoy problem solving are encouraged to apply today!

Medical Delivery / Patient Service Representative: We are seeking a mature, committed individual to deliver medication and supplies to patients throughout Ohio on an as-needed basis. This position is ideal for candidates with flexible schedules, as the Patient Service Representative could work part-time or full-time in a given week depending on our delivery and staffing schedules. Qualified candidates include high school or college graduates with excellent communication skills, a genuine desire to help others and a positive, self-starting attitude. Keen attention to detail is required. Additional requirements include a valid Ohio driver's license, a good driving record, current auto insurance coverage and the ability to lift a 50 pound box from the floor. Prior healthcare delivery experience is preferred.
